Program Name: Medicare Eligible Hospital/Critical Access Hospital (EH/CAH)

Measure Objective: Health Information Exchange

2024/2025 Promoting Interoperability Measure: Support Electronic Referral Loops by Sending Health Information:For at least one transition of care or referral, the eligible hospital or CAH that transitions or refers their patient to another setting of care or provider of care: (1) Creates a summary of care record using certified electronic health record technology (CEHRT); and (2) electronically exchanges the summary of care record.

Notes:

1: For 2023, the scenarios for the Medicare Promoting Interoperability measure require the EH to create and transmit/exchange a summary of care record and to confirm receipt of the transmitted/exchanged summary of care record and submit data for a continuous 90-day period within the calendar year.

2: For 2024, the scenarios for the Medicare Promoting Interoperability measure require the EH to create and transmit/exchange a summary of care record and to confirm receipt of the transmitted/exchanged summary of care record and submit data for a continuous 180-day period within the calendar year.

3: Test data supply: ONC and Vendor-supplied. ONC provides the Test Data Scenarios and parameters; the Vendor supplies the summary of care records within the parameters.

4: The name of this measure changed from Transitions of Care to Support Electronic Referral Loops by Sending Health Information. No other changes were made to the test data.

5: Starting in 2019, for the Medicare Promoting Interoperability measure a hospital may use any document template within the C-CDA for the purpose of the measure.
